However, honest mistakes, or policy decisions that were later seen to be inappropriate, do not fall into that category. Pursuing those who do their jobs and take decisions that are either later seen to be unwise, or don’t match your own, isn’t seeking accountability. It’s conducting a witch hunt.

Purely on a practical level to do so would be a hugely expensive knee jerk reaction to Musk’s interference that would deliver nothing and delay implementation.

Apart from the obvious that it’s none of his business Musk has been adopting a strategy solely based on misinformation and innuendo.

Therefore whilst it’s plausible that Afzal made an honest error it’s not that Musk did.
That doesn't make any sense. Afzal was told something and repeated it and that was an honest mistake. Musk was told something and repeated it and he's lying?

Holding those responsible for policy accountable is a witch-hunt? How convenient. Besides which this view is too subjective to be taken remotely seriously. Had it been Farage's decision you'd be all over it like a pigeon on a chip,
 
Should we endeavour to find those accountable ? #surely that is part of the process to find how this was allowed to happen.
Obviously the child rape networks bear primary responsibility, but in order to make situations like these preventable, we need to know who made what decisions, based on what evidence, or on whose authority. ?

And this should be devoid of politics. People are accountable, this is not "group think" policy to allow/facilitate child rape.
These slimy b'stads are still in their jobs or getting ring-fenced pension from their activities facilitating mass child rape. Or do we possibly think funds are being diverted to reward people for not breaking the silence ?

That’s like asking how long is a piece of string!

It’s impossible to know for certain but generally no I don’t.

Individuals might have covered up things but the more people are involved the less likely it becomes. Leaks and whistleblowers ensure that. I once worked for a company who were being so squeezed by the major supermarkets to cut the costs of the own label products they produced for them that they secretly amended the recipes so they no longer matched the labelling.

This was handled by a small group close to the CEO but unknown to people like me. It didn’t last long. The factory people saw new recipes. So did quality control. Some disaffected employees reported it to the authorities and all hell broke loose. Big fines resulted and the CEO lost his job. The company then withdrew from the activity for a while as it was unprofitable but when reasonable pricing resumed came back in.

I therefore think that serious cover ups are very unlikely. Far too risky.

Accountable for what?

Deliberately facilitating? Of course. They should and I am sure they would be if the evidence exists. The inquiry was charged with evaluating that.

If those decisions were taken based on ideology there should be some degree of accountability.
The same standards apply to all? Really? When those of whom you approve act in good faith and those of whom you don't are malicious liars that's just not true.
