It's an institutional failure, whatever way you look at it. Someone has signed off on this guy's training and signed him off as ready for frontline duty.

I kind of feel sorry for the guy. Painfully out of his depth and he doesn't want to be there any more than the suspect. He'd obviously be much happier in a nice, cushy customer service role somewhere. The institution is to blame for pushing him into it.
